Understanding Baxter's nerve entrapment

Baxter's nerve entrapment, also known as inferior calcaneal neuropathy, is a condition in which the first branch of the lateral plantar nerve becomes compressed or irritated as it passes through a narrow anatomical tunnel near the inside of the heel. This nerve, commonly referred to as Baxter's nerve, runs between the abductor hallucis muscle and the quadratus plantae muscle before traveling across the bottom of the heel to supply sensation and motor function to parts of the foot. When this nerve becomes entrapped, it can produce chronic heel pain that closely mimics plantar fasciitis, making accurate diagnosis challenging. Studies suggest that Baxter's nerve entrapment may account for up to 20 percent of chronic heel pain cases.

The anatomy of Baxter's nerve explains why it is vulnerable to entrapment. The nerve originates from the lateral plantar nerve, which is a branch of the tibial nerve, and it takes a sharp turn around the medial aspect of the heel as it courses from the inside of the ankle toward the bottom of the foot. At this turning point, the nerve passes through a tight space between muscles, fascia, and bone. Any swelling, thickening, or anatomical variation in this region can narrow the space further and compress the nerve. The nerve ultimately provides motor innervation to the abductor digiti minimi muscle, the small muscle on the outside of the foot that moves the little toe.

Several factors can contribute to the development of Baxter's nerve entrapment. Flat feet and excessive pronation cause the foot to roll inward during walking, increasing tension and compression on the nerve as it passes around the medial heel. Enlargement of the abductor hallucis muscle, which can occur in athletes or individuals who spend long hours on their feet, can narrow the tunnel through which the nerve travels. Heel spurs, chronic plantar fasciitis with thickening of the fascia, and swelling from other conditions can all impinge on the nerve. Varicose veins or other vascular abnormalities in the area have also been identified as potential contributors.

The symptoms of Baxter's nerve entrapment share many features with plantar fasciitis, which is why the two conditions are so frequently confused. Both conditions produce pain on the bottom of the heel that is typically worse with weight-bearing and activity. However, there are subtle differences that can help distinguish them. Baxter's nerve pain often has a burning or tingling quality, reflecting its neurological origin, whereas plantar fasciitis pain is more commonly described as a sharp or stabbing sensation. The pain from nerve entrapment may radiate toward the outside of the heel or the lateral aspect of the foot, following the course of the nerve.

Another distinguishing feature is the pattern of pain throughout the day. Plantar fasciitis classically produces the most severe pain with the first steps in the morning or after prolonged rest, with symptoms often improving after a few minutes of walking. Baxter's nerve entrapment, while sometimes producing morning pain, more characteristically worsens throughout the day with prolonged standing or activity and may not have the same dramatic improvement with initial movement. Some patients describe the pain as being present even at rest, particularly after a long day on their feet, which is less typical of pure plantar fasciitis.

Signs and Symptoms to Watch For

Physical examination by a skilled podiatrist can reveal signs suggestive of Baxter's nerve entrapment. Tinel's sign, which involves tapping over the course of the nerve to reproduce tingling or pain, may be positive along the medial heel. Tenderness may be noted in a more medial and plantar location than the classic insertion point of the plantar fascia at the medial calcaneal tubercle. In chronic cases, atrophy of the abductor digiti minimi muscle on the outside of the foot may be visible, as the compressed nerve loses its ability to adequately stimulate the muscle it supplies. This finding is highly suggestive of nerve entrapment.

Advanced imaging can help confirm the diagnosis of Baxter's nerve entrapment when clinical findings are suggestive but not definitive. MRI may reveal swelling or signal changes along the course of the nerve, thickening of the surrounding muscles, or atrophy of the abductor digiti minimi muscle. Ultrasound in experienced hands can sometimes visualize the nerve and identify the point of compression. Electrodiagnostic studies, including nerve conduction velocity testing and electromyography, can document abnormal nerve function, though these tests can be technically challenging to perform on small nerves in the foot.

Conservative treatment for Baxter's nerve entrapment begins with addressing the factors that are contributing to nerve compression. Multiple injections may be given over time, though the frequency is typically limited to avoid potential complications from repeated steroid exposure in the area.

When conservative treatment fails to provide adequate relief after several months, surgical intervention may be recommended. The surgical procedure for Baxter's nerve entrapment involves releasing the nerve from the structures that are compressing it, a procedure known as nerve decompression. This typically requires releasing a portion of the deep fascia of the abductor hallucis muscle and any other tight bands or structures that are impinging on the nerve. If a heel spur is present and contributing to the compression, it may be removed during the same procedure. The surgery can be performed through a small incision on the medial side of the heel.

Recovery from Baxter's nerve decompression surgery generally proceeds well, though patience is required as nerve healing is inherently slow. Most patients are placed in a protective boot or cast for two to four weeks following surgery and gradually transition back to regular footwear. Weight bearing is typically allowed relatively early, though activity is limited during the initial healing period. Improvement in symptoms may be noticed within weeks, but complete resolution of nerve-related symptoms can take three to six months as the nerve heals and regenerates.
