When an MRI Is Needed Instead of an X-Ray for Foot Pain

Two Tests That Answer Different Questions
Patients frequently arrive convinced they need an MRI, often because a previous X-ray was normal and the pain continued. Sometimes that instinct is exactly right. Often a different test would answer the question faster and at a fraction of the cost.
X-rays use ionizing radiation and image dense structures. They show bone shape, alignment, joint space narrowing from arthritis, spurs, dislocations, and most acute fractures. Critically, standing weight bearing X-rays show how the bones relate to each other under body weight, which is how the foot actually functions.
MRI uses a magnetic field and radio waves rather than radiation, and it images soft tissue and the interior of bone with detail nothing else matches. It shows tendons, ligaments, cartilage, nerves, tumors, and bone marrow changes that are invisible on X-ray.
That last capability is the one most patients do not know about. An MRI sees inside bone. Bone marrow edema, meaning fluid within the bone itself, appears clearly and is the earliest sign of a stress reaction before any visible fracture line has formed.

This explains the most important limitation of X-rays in foot care. A stress fracture typically does not appear on X-ray for two to six weeks after symptoms begin, because it takes that long for enough bone resorption or healing callus to become visible. An X-ray on day three of a genuine stress fracture is very often entirely normal.
When a Normal X-Ray Is Misleading
Acting on that normal X-ray by returning to activity is how a stress reaction becomes a complete fracture. When the history and examination strongly suggest a stress injury, treatment should begin regardless of the X-ray, with MRI reserved for cases where the diagnosis genuinely changes management.
Certain locations warrant lower thresholds for advanced imaging because the consequences of a missed injury are severe. The navicular, the base of the fifth metatarsal, and the medial malleolus all have marginal blood supply, and stress fractures there are prone to nonunion if treated as ordinary soreness.

Lisfranc injuries are another situation where standard imaging misleads. A subtle midfoot ligament injury can look normal on a non-weight bearing X-ray and become obvious on a weight bearing view, because only under load do the unstable bones separate. Missing this injury leads to lasting midfoot arthritis.
CT scans fill a role that neither test covers well. CT shows bone in fine three-dimensional detail, making it the best choice for complex fractures, assessing whether a fusion has healed, and evaluating small bones such as the sesamoids where overlapping structures obscure plain films.
Why Imaging Should Follow the Examination
For superficial soft tissue questions, ultrasound frequently outperforms MRI in practical terms. It is immediate, inexpensive, and dynamic. Plantar fasciitis, most tendon problems, Morton's neuroma, and ganglion cysts can usually be assessed without ever ordering an MRI.

MRI has real drawbacks beyond cost. It is time consuming, uncomfortable for patients with claustrophobia, unavailable to those with certain implanted devices, and often requires insurance authorization that adds days or weeks. It also detects incidental findings that have nothing to do with the symptoms and can lead to unnecessary treatment.
The sequence that works is examination first, imaging second, and only the imaging that answers a specific question raised by the examination. Imaging ordered before anyone has formed a clinical impression tends to produce data rather than answers.
